Muscle Relaxing Agents / Anti-Wrinkle Injections
Smiles, frowns, and other facial expressions are controlled by the facial muscles. The loss of skin elasticity that occurs with ageing means that the underlying muscle movement produces wrinkles which worsen over time. Common sites are the 'frown lines' in the glabella (the area between the eyebrows), 'crow's feet' at the outer aspects of the eyes, and horizontal lines and creases on the forehead due to the action of the frontalis muscle which raises the eyebows. In a good many patients this muscle is unconsciously overused to elevate drooping skin in the upper eyelids ( blepharochalasis ). In addition to the forehead wrinkles this can produce 'tension headaches'. Muscle relaxing injections in the forehead can thus not only eliminate the wrinkles but also relieve the headaches. Similarly upper blepharoplasties can help the forehead muscles to relax, as there is no longer any excess skin to be elevated.
Muscle relaxing agents contain a neuro-muscular toxin (botulinum toxin) that causes muscles to become temporarily paralysed. Following injection into targeted areas, the underlying facial muscles cease to contract for approximately 4 mths. As a result, skin wrinkles produced by activity of underlying facial muscles can be eliminated. Results are usually noticeable within 2 - 3 days of treatment. Treatment needs to be repeated every four to six months.

Injectable Fillers
Unlike muscle relaxing injections, injectable fillers smooth away wrinkles and lines by adding volume to the skin and the deeper tissues. Several of the most popular fillers are made of hyaluronic acid and are synthesised in the laboratory and therefore don't carry the risk of an allergic reaction such as used to be a problem with the original fillers made from collagen derived from cows. Patients initially may not like the fact that the treatment is not permanent and has to be repeated twice a year. However permanent fillers carry the risk of irreversible cosmetic deformity due to wrong placement, migration of the filler, acute and chronic infection and extrusion.
